Consent is usually required from a parent/carer with parental responsibility before this information can be shared. Consent is not required in some instances. GUIDANCE ABOUT CASS LEVEL 2 INFORMATION REQUESTS
Do not use this form if you know the child has an allocated Social Worker/Family Support Worker. Please contact the worker directly if you have their contact details. If you know that the child has an allocated social worker/Family Support Worker but do not have the worker’s contact details please call 0121 303 1888. Select Option 2 Option 1 and this contact information will be shared with you once your identity has been verified.
If you have concerns about a child, you need to submit a Request for Support Form and not this form. The Request for Support Form can be completed and submitted online at (link to the online Request for Support Form).
If you are:
- a parent/carer who has parental responsibility wishing to access information from your child’s social care record,
- or an adult wanting information from your historical record held by children’s services when you were a child /young person.
