Referrals must be in writing using the referral form below:

Once you’ve completed the form, email it to:

Please be aware that the CYPOT service uses email to get in touch with you. We will send all confirmations, responses and information via email.

What happens next?

Once we've received your referral form, it will be reviewed and prioritised relating to child protection and end-of-life care.

• You'll be notified by letter that we've received your referral form
• The parent or carer will be contacted to arrange an assessment
